Summary

  • Consolidate workflow, planning, review-delivery, security, and orchestration guidance with structured contract, ledger, and evaluation validation.
  • Harden mise-managed skill-evaluation migration, preparation, historical, and materialized-artifact checks, with focused fixtures and documentation updates.
  • Harden review-watch staged state recovery, activity/grace tracking, lock portability, paginated thread resolution, legacy-head handling, and focused tests.
  • Clarify credential policy: alphabetic prose after real Bearer/Basic schemes passes; long lowercase/uppercase hexadecimal and mixed-class credential values fail; redacted controls pass.
  • Require plan-preserving recovery to use present, consistent, actionable plan fields; missing or non-actionable fields require a targeted amendment and new plan version.
  • Align recovery evals so stories are blocked/frozen and evidence is preserved before workflow-policy freeze/report/escalate/stop, with planning recovery afterward.

Validation

  • Focused workflow-validator, skill-evals, and review-watch tests
  • JSON, shell syntax, Prettier, rumdl, and git diff --check
  • mise run ai:skills:evals:validate --skills-path home/.agents/skills
  • mise run fix
  • mise run check
  • Pre-commit hooks, Betterleaks, and commitlint

Reviewer notes

  • Review-watch preserves explicit null resolution state when GitHub cannot prove a thread status; downstream consumers must treat it as unresolved or unknown.
  • Generated evaluation artifacts outside this amendment remain untouched.

Summary by CodeRabbit

  • New Features

    • Added guidance for planning, delegating, executing, reviewing, and delivering multi-step work, with clearer authorization, recovery, and handoff boundaries.
    • Added workflow contract and evidence-ledger validation, plus evaluation workspace migration and validation.
    • Added more structured critique modes with decision tracking and bounded autonomous review cycles.
    • Shell startup now selects the repository’s mise configuration by default while preserving explicit settings and supporting opt-out.
  • Improvements

    • Expanded review-watch tracking for changes, unresolved feedback, check status, and interrupted operations.
    • Added evaluation coverage for the new workflow, security, review, and migration guidance.

Walkthrough

This pull request revises workflow guidance, adds workflow contract and ledger schemas with a structural validator, and updates evaluation migration and validation. It also changes Zsh startup behavior to select repository mise configuration under specified conditions.

Changes: Workflow Skills and Validation

Layer / File(s) Summary
Workflow contract and ledger validation
home/.config/mise/tasks/ai/workflow/*, README.md, tests/scripts/test_mise_task_ai_workflow_validate.zsh, tests/scripts/README.md
New schemas define workflow contracts and ledgers. The validator checks ledger structure, paths, identities, transitions, dependencies, and redaction. Delivery records report external authorization and runtime requirements.
Coordination and worker policy
home/.agents/skills/workflow-policy/*, home/.agents/agents/orchestrator.md, home/AGENTS.md
The new policy defines delegation, worker contracts, checkpoints, coordination, and recovery. Orchestrator guidance and the skills index refer to the new policy and workflow owners.
Planning and story execution
home/.agents/skills/planning/*, home/.agents/skills/execution-workflow/*
Planning produces a persisted accepted plan. Execution uses its exact path and version and defines story states, dependencies, acceptance, and drift recovery.
Critique and security procedures
home/.agents/skills/critique-workflow/*, home/.agents/skills/security/*
Critique guidance defines findings, decision logs, and autonomous and interactive modes. Security guidance defines routing, redaction, finding dispositions, and closure evidence.
Pull request review and delivery
home/.agents/skills/review-delivery/*, home/.agents/skills/pr-orchestration/*, home/.agents/agents/orchestrator.md, home/AGENTS.md
The new review-delivery skill defines review, finding closure, watch, and authorized delivery procedures. The prior PR orchestration skill and evaluation file are removed.
Skill evaluation migration and validation
home/.config/mise/tasks/ai/skills/evals/*, home/.config/mise/cleanup.paths, tests/scripts/test_mise_skill_evals.zsh
A migration map records renamed skills and legacy preparation skills. The evaluation validator handles migration names, workspace states, reports, and evidence paths; tests cover the new validation rules.

Repository mise Startup

Layer / File(s) Summary
mise startup selection and tests
home/.config/zsh/.zshenv, tests/scripts/test_mise_config.zsh
Zsh startup sets repository mise configuration variables when repository configuration exists, unless opted out, and preserves values set by the caller. Tests cover these conditions.

Change: Feature

Merge Risk: Moderate · up to c332e

Restore the missing evaluation artifacts before merging, or explicitly accept the resulting validation failure. Alternate-root validation, recovery guidance, and macOS portability also retain bounded limitations.

Security architecture risk: Low · up to e5878

The changed guidance preserves authorization, isolation, and review requirements, and the structural checker refuses to authorize delivery. Shell configuration overrides and an opt-out remain available. Runtime authorization, cancellation, and recovery enforcement are outside the reviewed implementation, leaving some uncertainty.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The startup authority change affects subsequent mise processes inheriting the exported environment, not just commands run inside this checkout. Its effective authority is that of the invoking local user. Exploiting the selected configuration would require influence over the fixed repository configuration path or another trusted configuration input; ownership of that path was not established.

Trust Boundaries and Controls

  • observed — Before reporting valid structure, the checker rejects designated trusted-status claims, noncanonical or sensitive path claims, overlapping candidate claims, inconsistent identities, and discontinuous transitions. Worktree identifiers remain opaque strings: validation does not resolve them, verify authorization provenance, or establish permission to access a filesystem target.

Resilience and Maintainability Implications

  • inferred — Exact accepted-plan and amendment identity is required by planning and execution guidance, but the inspected recovery records and identity comparison do not themselves prove that each replacement worker or completion result used that accepted version. Likewise, exclusive ownership after interruption depends on external enforcement. This is an evidence limitation, not an observed unauthorized transition.

Hardening Proposals

  • proposed — When integrating an external orchestrator, bind recovery handoffs and completion evidence to the accepted plan version and current ownership generation, and verify those bindings together with fresh authorization before resumed mutation.

Unverified finding
We couldn't verify this from the code. It is shown for review, not as a verified issue.

The record-order reduction starts with no previous checkpoint for a candidate, so it may accept its first record even when that record predates the candidate’s contract checkpoint.

Validation may certify a record chronology that precedes its initiating contract.

This can occur when: A contract checkpoint is dated in 2026 and its first record checkpoint is dated in 2025, as the new positive chronology fixture permits.

We couldn't verify this from the code. It is shown for review, not as a verified issue.

The positive chronology test dates records in 2025 while its baseline contract uses the current date. It may require accepting records before their contract if contract checkpoints establish the chronology baseline.

This PR’s positive test fails despite the validator correctly rejecting the fixture.

This can occur when: Contract-to-first-record chronology is enforced.

Test gap: the --base-path home assertion does not create a workspace that needs a migration mapping, so it passes even if the task-source map is never loaded.

The test reports successful migration-map resolution without exercising it.

This can occur when: Map lookup regresses for --base-path home while the existing home workspaces need no mapping.

Unverified finding
We couldn't verify this from the code. It is shown for review, not as a verified issue.

Test gap: the invalid fixture moves all checkpoint fields backward, so a broken validator comparing only initial_at would reject it and pass the test.

The suite fails to catch acceptance of out-of-order checkpoint fields.

This can occur when: A later record increases initial_at but decreases last_at or next_due_at.

Test gap: the new cross-contract path cases assert only accepted exclusions. A validator that never checks overlapping owned_paths between candidates could still pass them.

The test suite would not detect acceptance of two candidates owning the same repository path.

This can occur when: The cross-contract ownership check is removed or accidentally bypassed.

With valid timing reports now able to pass the corrected key check, validate_materialized_config checks timing and grading run_id values separately but does not bind them to the same run.

Validation may accept a grading result as evidence for a different timed execution.

This can occur when: A configuration has timing.json for run-1 and grading.json for run-2, each otherwise well formed.

The repaired materialized timing check makes the evidence-validation path reachable; that path invokes realpath -P, which may be unsupported by the macOS realpath selected in a supported installation.

An otherwise valid materialized workspace fails evidence-path validation.

This can occur when: A materialized iteration with output or evidence references is validated on a supported macOS setup without GNU-compatible realpath first on PATH.

An explicitly historical iteration under a migration-source workspace is classified as historical even when case artifacts are present, bypassing the later configuration-report and evidence-file validation.

Validation may certify historical evaluation artifacts that could not pass the report checks applied to other artifact-bearing iterations.

This can occur when: A migrated workspace declares state: historical and includes case directories containing incomplete or inconsistent materialized reports.

Iteration-level preparation summaries are checked for JSON and identity, but not for fabricated execution results; the preparation-specific validation later checks only configuration timing and grading files.

The validator can report a preparation workspace as valid despite execution claims in its summaries.

This can occur when: A state: preparation iteration retains null configuration timing metrics but its benchmark.json or feedback.json claims a passed measured run.

Parsing the three timestamps does not make the subsequent array comparison component-wise: jq can accept a later record whose initial_at increased while last_at and next_due_at went backward.

The validator reports chronologically inconsistent checkpoints as valid.

This can occur when: Two otherwise valid records for one lane and candidate have checkpoints [00:00, 00:10, 00:20] and [00:01, 00:02, 00:03], respectively.

Test gap: every standalone bearer credential in the rejection list uses mixed character classes, so the tests can pass while lowercase hexadecimal bearer credentials evade credential_token.

The secret-regression test remains green despite that unredacted credential being accepted.

This can occur when: The validator accepts Bearer 0123456789abcdef0123456789abcdef in an acceptance string.

Test gap: the materialized fixture gives timing and grading identical run_id values and has no negative pairing case. A validator that checks each ID’s syntax but not their equality still passes.

The suite does not catch acceptance of reports from different runs as one materialized configuration.

This can occur when: One configuration’s grading report is changed to run-2 while its timing report remains run-1.

The loaded rename map supports iterative resolution and checks cycles, but the later target check requires every intermediate rename target to have an active evaluation spec while active migration-source names are forbidden. A valid chain therefore appears impossible to validate.

Validation rejects historical workspaces that should resolve through both renames.

This can occur when: The map contains old-a -> old-b -> current, with only current an active skill.

For project-root and home base paths, map selection ignores the repository-local map and silently proceeds if the task-adjacent map is missing. A partial or mixed-version task installation may therefore lose required migration metadata.

Valid historical workspaces can fail name resolution, or map-dependent preparation classification can fail.

This can occur when: The validator script is installed without its adjacent migration-map.json, while the repository being validated has that map and migrated workspaces.

credential_token requires three character classes, so a long lowercase hexadecimal bearer credential in a nonsensitive free-text field can pass the new standalone bearer/basic check.

A ledger containing an unredacted credential may pass the secret check and be shared as clean.

This can occur when: An acceptance string contains Bearer 0123456789abcdef0123456789abcdef without an Authorization: prefix.

The four-check grace period specifies 15-minute intervals but leaves cadence to an external scheduler; a poll-count-based stop signal could complete the grace period early.

Review monitoring stops before the intended grace interval has elapsed and can miss feedback arriving during that interval.

This can occur when: An external scheduler retries or polls a clean, approved open PR four times within a few minutes.

The material-boundary assertion accepts a planning amendment described as created and accepted without requiring the amendment critique mandated by planning/SKILL.md.

The evaluation can approve a response that skips the required critique gate.

The no-amendment recovery path can retain an accepted plan with missing or non-actionable required story fields, even though those fields caused execution to block.

Execution resumes at research, validates the same unusable story, and blocks it again; its dependents remain blocked.

This can occur when: A frozen plan lacks a required story key or rollback note, but its objective and listed material boundaries remain unchanged.

The drift procedure can have planning accept an amendment after the critique at line 223, before the separate critique requested at line 230 and the instruction at line 231 to wait for critique before changing the plan.

Recovery can proceed from an amendment that conflicts with the later critique, or stall while workers reconcile two critique decisions.

This can occur when: The first critique recommends a boundary change, planning accepts an amendment, and the later critique reaches a different keep-or-discard decision.

The new viable-recovery evaluation expects no amendment for unchanged boundaries, while the existing mapping-story-states-to-todos evaluation requires a targeted amendment after an excluded-file edit and missed checkpoint without stating that a plan boundary changed.

The evaluation set can reward amendment of an unchanged plan in one case and reject it in the other.

This can occur when: The excluded-file edit and missed checkpoint are worker violations, but the accepted plan and its material boundaries remain viable and unchanged.

Scheduler shutdown now depends exclusively on pr:review-watch -- poll emitting scheduler_should_stop: true, including after merged or closed PRs have been removed; the producing task's behavior is not supplied.

If an empty-state poll omits the signal or cannot run, the external scheduler has no permitted shutdown signal and continues polling indefinitely.

Story budget recovery permits resumption after accepted plan recovery without repeating the earlier requirement to block a lane lacking external runtime enforcement.

The recovered story may resume work without enforceable hard budgets.

This can occur when: A story exhausts its budget while its runner's external budget enforcement has become unavailable, and planning accepts an unchanged-plan recovery.

Staging an add leaves state.pull_requests unchanged until replay, while the unchanged list action reads only that array and does not report or acknowledge pending_add. A list between staging and acknowledgment can say the watch is empty when an add is awaiting delivery.

The list consumer may stop scheduling or conclude that the PR was not registered, leaving the staged add unacknowledged.

This can occur when: An add is interrupted after staging a PR into an initially empty watch list, and a scheduler or operator invokes list before retrying add.

The staged-add branch accepts an add invocation with no valid PR numbers and replays and commits the previous add, bypassing the normal add requires at least one pull request number check.

Add reconciliation sets action_required from checks and CHANGES_REQUESTED alone, unlike poll results, which include new activity. Existing review or comment findings can therefore be present in reconciliation while its action flag is false.

A consumer that gates review work on action_required can skip the very findings the add reconciliation was intended to surface.

This can occur when: An approved PR is added with existing comments or review findings and no failed or pending checks.

A poll payload is emitted before its staged state is cleared, so an interruption after successful emission causes the same payload to be emitted again on retry. The change establishes at-least-once delivery, but the supplied code does not show acknowledgment or idempotency by every output consumer.

Without downstream deduplication, the same review activity can prompt duplicate findings, responses, or other delivery actions.

This can occur when: A consumer receives and acts on poll output, then the watcher exits before the final saveState; the consumer retries the poll.

mergeSeen permanently retains every previously observed ID, including IDs no longer present in either fetched collection. On a long-lived PR with substantial comment churn, each poll grows or rewrites a state file containing the full historical ID set.

The state file and repeated serialization and deduplication work can grow without a bound tied to currently returned activity.

This can occur when: A PR receives many reviews or comments over an extended watch period, including comments later deleted or no longer returned.

After moving a lock to quarantine and discovering it is not the stale lock previously observed, restoration can fail with EEXIST and that failure is ignored. Another invocation can then hold the original lock path while its live predecessor's lock remains quarantined.

The fresh lock owner and the third invocation can both proceed against the same watch state, causing lost updates or duplicated transactions.

This can occur when: Concurrent invocations race to recover a stale lock: one acquires a fresh lock before the other renames it to quarantine, and a third acquires the newly freed lock path before restoration.

Both invocations may mutate the watch state concurrently, losing a staged operation or activity progress.

This can occur when: Two watch invocations share a state file but launch with different TZ or locale settings while the first still holds its lock.

Pending-poll validation checks only that payload is an object and the next entries are valid; it does not check the payload's action, repository, PR results, or relationship to those entries. A malformed persisted stage may be emitted and committed as if it were a valid poll.

A retry can report incorrect poll results and advance the watch state despite never delivering the corresponding activity.

This can occur when: A pending state file contains a valid entry array but an incomplete or mismatched payload, for example after manual recovery or a state-writing component produces an incompatible record.

Grace completion depends on the current clean boolean and retains no requirement that previously reported findings have been resolved. An approved PR with an outstanding older review comment can accumulate four clean polls and become eligible for scheduler shutdown.

formatComment derives resolution status solely from fields on comments fetched through gh api .../pulls/{number}/comments. If that REST response has no thread-resolution field, is_resolved is omitted from the JSON output even though review delivery may need it to close findings.

A consumer relying on this reconciliation or poll output may treat an unresolved finding as closed or be unable to verify its closure.

This can occur when: A PR has an unresolved review thread whose comments are returned without isResolved or resolved.

A retried staged poll emits its saved stop decision and commits its saved next entries without fetching current PR activity. A stop payload computed before an interruption can remain authoritative after new review activity arrives.

The retry can tell the scheduler to stop while the new review has never been reported.

This can occur when: The fourth clean poll is staged and interrupted; a new review arrives before the poll is retried.

Test gap: the failed malformed-review poll redirects stdout to __poll_output but never asserts that it is empty. A poll that emits findings before discovering the malformed timestamp could pass the new failure and state-integrity checks.

The test accepts partial output from a failed, unacknowledged poll.

This can occur when: Valid activity is processed and written to stdout before a later review fails timestamp validation.

Unverified finding
We couldn't verify this from the code. It is shown for review, not as a verified issue.

Test gap: the reconciliation assertions check comment bodies and linkage but not the inline or issue comment's own ID or URL. An add implementation that drops those fields from its output could satisfy these assertions while still persisting the seen IDs.

The complete-reconciliation test accepts comment output that a review recipient may be unable to identify or link back to GitHub.

This can occur when: The add result contains the supplied comment bodies but omits id and html_url from either comment entry.

The two background contenders have no barrier requiring both to reach lock acquisition before either finishes. A correctly serialized implementation can therefore produce two successful, sequential polls and fail the exactly-one-winner assertion.

The test fails intermittently even though neither poll overlaps a live lock holder.

This can occur when: Scheduling delays the second background job until after the first has released the lock.

Test gap: every poll in the four-check grace sequence is clean. A counter that increments on clean polls but fails to reset after new review activity could pass this test.

The scheduler may stop before four consecutive clean checks have occurred, without this test detecting it.

This can occur when: A review or comment arrives between clean polls, followed by enough clean polls to bring the old, unreset count to four.

Test gap: the final assertion only requires a non-null lastSeenAt, but the initial PR already has one. Recovery that replays the payload and deletes pending_poll without applying next_pull_requests passes it.

The test reports successful recovery although the staged cursor was not advanced.

This can occur when: Pending-poll recovery leaves lastSeenAt at 2024-01-02T00:00:00Z instead of applying the staged 2024-01-03T00:00:00Z value.

Unverified finding

Possible test gap: both new poll-side comment fixtures are empty. If other poll tests do not supply and assert nonempty pages, a regression that ignores inline or issue comments during poll would pass despite the nonempty add-path coverage.

Poll output omits new review activity without this fixture exposing the regression.

This can occur when: A watched PR receives a new inline or issue comment on a later poll, while the poll implementation skips that comment endpoint or its returned entries.

Test gap: staged-add recovery checks only seen.reviews, although the initial add checks all three seen arrays. Recovery that loses seen.inline_comments and seen.issue_comments could pass; the crash-recovery assertion at lines 192–193 has the same omission.

The recovery assertions pass with incomplete watch state; a later poll may report the already-reconciled comments again.

This can occur when: A retry replays pending_add.payload and restores the review cursor, but does not restore the staged inline- and issue-comment cursors.

The fake ps delegates to /bin/ps instead of the ps found on the runner's PATH. On a supported runner without /bin/ps, checks of any PID other than the specially handled stale PID fail for a reason unrelated to lock correctness.

The lock test may fail or make the wrong contention decision solely because its fake process checker cannot delegate.

This can occur when: The concurrency test runs on a platform with ps available through PATH but no /bin/ps, and lock recovery checks a live contender's PID.

Add assertions for the parallel start of Unit B.

File: home/.agents/skills/workflow-policy/evals/evals.json

The parallel-branch-supervision case directs Unit B to start against the agreed stub before integration. The case has no assertions array, and its recorded grading contains zero checks. A change that serializes Unit B behind Unit A would not change any recorded result. The evaluator accepts cases without assertions, so this is a coverage gap for a stated policy, not a validation failure.

Keep the case artifacts for materialized eval iterations.

The validator reads each manifest state. For a materialized iteration, it requires each case directory, its outputs/ directory, timing.json, and grading.json. When those are absent, the run stops with "evaluation iteration has no case artifacts or explicit historical state."

These iterations set "state": "materialized" but do not track their case artifacts:

  • .agents/eval-workspaces/review-delivery/iteration-1
  • .agents/eval-workspaces/review-delivery/iteration-2
  • .agents/eval-workspaces/review-delivery/iteration-3
  • .agents/eval-workspaces/workflow-policy/iteration-1
  • .agents/eval-workspaces/workflow-policy/iteration-2

Commit the complete redacted case artifacts for these iterations. Keep the state as materialized. Do not set the state to historical or preparation to pass validation, and do not weaken the validator's checks.

This finding is acknowledged but intentionally not changed in this delivery. The affected review-delivery and workflow-policy materialized evaluation workspaces are pre-existing generated artifacts outside the four accepted patch scopes, and the execution handoff explicitly says not to modify that unrelated area to make repository-wide validation pass. The focused changed-scope tests, mise run fix, mise run check, Betterleaks, and normal pre-commit hooks pass. Leaving the thread open for a separate artifact-generation change.

Count the raw nested comment artifacts before you classify findings. Treat each reply as evidence for its parent unless it adds a distinct finding. If the reported count differs from the collected artifact count, block disposition. Record an incomplete review poll.

Delegate the complete review poll and candidate validation to a review-monitoring worker. The worker must inventory every review artifact, reconcile each finding, and run bounded validation. Consume its evidence instead of repeating the same review scope in the orchestrator.

Gate scheduler_should_stop on unresolved review threads.

scheduler_should_stop can become true while an approved PR still has an unresolved review thread. The clean check ignores thread resolution state, so four quiet polls complete the grace period. Include unresolved review threads in clean, so the grace counter resets while any finding remains open.

Acknowledge poll output before advancing the watch cursor.

pr:review-watch -- poll saves the next cursor and clears pending_poll right after it emits the findings. If the delegated worker stops before it writes those findings to the ledger, later polls do not return them, and add skips watched pull requests. Add an acknowledgement step to pr:review-watch. Keep pending_poll until the worker confirms the ledger write. Then update the worker instruction to acknowledge only after that write.

The thread-resolution portion is addressed in 199ab93: poll resolution lookups are bounded, unresolved or unknown inline threads make has_unresolved_activity true, reset the grace counter, and prevent scheduler_should_stop until four clean checks complete. The separate ledger acknowledgement request is an external consumer-boundary change not in the accepted four-patch plan; this task intentionally preserves at-least-once replay and leaves cursor acknowledgement to downstream consumers. Focused review-watch tests, mise run fix, mise run check, and pre-commit hooks pass. Leaving this combined thread open because the second request is not fully implemented.

@minusfive Thanks for addressing the thread-resolution portion in 199ab93 and reporting the passing checks.

I understand that ledger acknowledgement is outside the accepted four-patch plan. That concern remains unresolved; replay of poll output does not by itself confirm a durable ledger write. The combined thread should remain open, as you requested.

Would you like me to open a follow-up issue for the downstream consumer acknowledgement boundary?

Check staged poll entries against the active watch list.

loadState and saveState validate a staged poll only against its own payload and next list. Neither checks that every active watch appears in the staged results. If the state file is edited so that an active PR is missing from the staged poll, acknowledging the poll saves the next list without that PR and stops watching it.

The current producer does not create this state. Add and remove are blocked while a poll is staged, so the active list cannot change between staging and acknowledgment. Passing the active list into the staged-poll check, and requiring each active number to appear in payload.pull_requests, would reject such a file before replay.

Name plan-level fields in the amendment rules of workflow-policy and planning.

workflow-policy and planning name only story fields. execution-workflow requires an amendment when any required plan field is missing. A plan that lacks a plan-level section, such as rollback, can still receive a same-version recovery decision under workflow-policy and planning. execution-workflow rejects that decision, so the story stays blocked until an amendment is accepted. Recovery is possible, but the two skills disagree. execution-workflow already has the correct rule and needs no change.

Add a positive case for a redacted authorization ledger.

The Authorization: ****** case uses a ledger with redaction_status: "clean". The validator rejects it through the redaction-status check. No case in this file accepts a ledger with redaction_status: "redacted". Add a case that sets redaction_status to "redacted" and acceptance to ["Authorization: ******"], and expect the validator to accept it. Rename the existing case so its name states that it covers a clean ledger that contains a redaction marker.

Add an assertion that requires drift critique before the planning decision.

The amendment-boundary-recovery case checks the freeze ordering and the amendment path, but no assertion requires the separate drift critique of the worker's report, changes, and validation evidence. The SKILL.md drift-recovery section requires that critique before planning decides viability. A response that skips the critique can still pass every listed assertion, including the unchanged-viable path.

Cover GraphQL-resolved review threads in the poll tests.

The gh api graphql stub always returns isResolved:false (tests/scripts/test_mise_task_pr_review_watch.zsh:52-56). No test returns a resolved GraphQL thread. Every GraphQL-backed inline thread therefore resolves to unresolved. The poll path's hasUnresolvedActivity check and grace progression are never exercised with a resolved value. The PR 10 assertion checks only action_required == true, which holds either way. The clean-grace test uses PR 11, which has no inline comments.

Make the stub's resolution value configurable. Shape its response to match the GraphQL query that threadResolution sends. Add a poll test with an inline comment that resolves through GraphQL. Assert that this thread no longer blocks a clean poll and that grace checks advance to completion.

Cover a same-ref, different-OID head change in the test.

The head-change poll changes only headRefName and sets no headRefOid. pullRequestHead in home/.config/mise/tasks/pr/review-watch.ts prefers headRefOid. Reject duplicate skill names across skill roots, or select the spec by source root.

When a skill name exists in both the primary root and an alternate root, validation always uses the primary spec. An iteration that prepare.zsh --skills-path created from the alternate spec then fails the current-manifest comparison. Either reject the duplicate name with a clear error, or record the source root in manifest.json and select the matching spec.

Add a collision test for same-named skills in both skill roots.

The alternate-root test creates global only under alternate/.agents/skills. The primary root has no global skill, so validation never resolves a name that exists in both roots. The collision branch in validate.zsh prefers the primary spec (primary_spec at lines 510-511) and falls back to the alternate spec only when no primary spec exists. A change that reverses or drops that preference can pass the current tests. Add a fixture where both roots define the same skill with different evals.json files, then assert which spec validation uses.

Add an assertion that execution resumes with the exact accepted amendment path and version.

The material-boundary eval requires an accepted amendment before resumption, but no assertion requires execution to use that amendment's exact path and version. We couldn't verify this from the code. It is shown for review, not as a verified issue.

Test gap: the changed expected output says to follow workflow-policy for worker recovery, but none of this evaluation's assertions requires its canonical freeze -> report -> escalate -> stop sequence. A response could proceed from blocking the story directly to amendment and resumption.

The evaluation may report a passing answer that skips the required worker recovery procedure.

This can occur when: An evaluated response addresses the excluded-file change and missed checkpoint by blocking the story, preserving findings, obtaining a critiqued amendment, and resuming, but omits the worker recovery sequence.

Require the unblock condition in the viable recovery eval.

The viable-recovery-without-amendment case asserts that the story is blocked or frozen with its evidence preserved. It does not assert the blocker reason or unblock condition. A response can block the story without an unblock condition and still pass every assertion. The sibling material-boundary-recovery case checks both fields, so the viable case should check them too.

Suggested fix
"The response blocks/freezes the affected story and preserves its evidence before workflow-policy begins the canonical freeze -> report -> escalate -> stop sequence.",
+ "The response records the blocker reason and unblock condition when it blocks or freezes the affected story.",
"The response requires the exact freeze -> report -> escalate -> stop sequence before the plan-preserving recovery decision.",

"expected_output": "Before workflow-policy begins the canonical freeze -> report -> escalate -> stop sequence, the affected story is blocked/frozen and its evidence is preserved. After workflow-policy completes that exact sequence, ask planning to record a plan-preserving recovery decision without an amendment or new plan version only because all required plan fields are present, consistent, and actionable and the material boundaries remain unchanged. Resume the story at `research` only after that decision, using the existing lifecycle.",

Update the assertions for the viable-recovery-without-amendment case in
evals.json to require that the response records both the blocker reason and the
unblock condition when blocking or freezing the affected story.
