refactor: split the document model and file loading out of runner - #153

The README repeated most of the book. Keep the feature list, install, a minimal usage example and links; add the missing pages to llms.txt.
Move Section, CodeBlock, LoopSpec, ParamDef and the meta types into section.rs, and file loading, imports and parsing into runner/loader.rs. Behaviour is unchanged.
