docs: catch docs up with the code for v1.5 - #137

Why: pre-ship audit found docs behind the code (v1.5 features missing, wrong uninstall and update-notice wording, stale lists). Impact: README, GUIDE, CONFIGURATION, plugin READMEs and /plugin descriptions; file links now say the terminal opens the file (Claude Code ignores ctrl/alt+click, checked in tmux).
