feat(engine): operation diagnostics and error scope for hosts - #577
During the hosted gateway's D1 overload incident (Oct 22–25), PostHog could not answer basic questions:
which workspace or caller a failing request belonged to
which engine operation was running
how long the expensive operations (inbox read, delivery listing, workspace delete, invocation sweep, mention backfill) took
Engine errors carried only path, method and request_id, and nothing timed engine operations.
What
EngineDeps.diagnostics?: EngineDiagnostics (new ports/diagnostics.ts, exported from @relaycast/engine/ports):
operation(sample) reports { label, workspaceId?, durationMs, outcome, errorName? } for inbox.get, deliveries.list, workspace.delete, invocations.sweep and backfill.mention_fts. errorName is error.name only, never the message.
progress(sample) reports { operation, workspaceId?, phase, table?, rows?, remaining?, durationMs? }. The mention backfill reports a batch per page and completed; the bounded workspace delete PR will use it for its cascade.
Callbacks are isolated (a throwing sink never affects a request) and absent by default.
Operation scope: labelled operations run inside an AsyncLocalStorage scope.
currentEngineOperation() lets a host's database wrapper attribute each statement's real cost (D1 rows_read/rows_written) to the operation in flight. The engine can't see that through drizzle.
configureEngineDiagnostics() covers maintenance callers that never build the app; createEngine configures it from deps.diagnostics when supplied.
Error scope: an error thrown inside an operation carries a non-enumerable label. app.onError adds workspace_id, agent_id, engine_operation and the matched route template (never a raw path) to its error log and captureException context.
How the operations are wrapped: each instrumented function is now a thin labelled wrapper around its unchanged body.
The interface was agreed with the session instrumenting the hosted worker (relaycast-cloud): the engine reports operation-level timing, and the worker reports statement-level D1 cost keyed by currentEngineOperation().
Tests
Unit: duration and outcome, scope propagation across awaits and timers, innermost-wins nesting, error labelling without leaking the message, a throwing sink being harmless, and the no-sink no-op.
Conformance through the real app:
inbox and ?status= delivery reads are labelled with their workspace
a failing inbox read reports workspace_id, agent_id, engine_operation: 'inbox.get' and route: '/v1/inbox' to captureException
the backfill reports progress through to completed
npm run typecheck, npm run lint, the engine suite (1,526 tests) and npm run build all pass.
Additive (Minor). REST and WebSocket contracts are unchanged.
The engine adds optional diagnostics sinks for operation timing and cleanup progress. It scopes diagnostics to requests, instruments selected engine operations, and adds available workspace, agent, operation, and route context to unhandled server-error logs and 5xx exception telemetry.
Request scope and error telemetry packages/engine/src/engine.ts, packages/engine/src/__tests__/conformance/engineDiagnostics.test.ts
Runs requests in the app’s diagnostics scope. Adds available request and operation context to unhandled-error logs and 5xx exception telemetry. Tests cover error context, including non-Error rejections.
A rabbit checks the inbox rows, And times the work as each batch goes. The labels follow errors through, While sinks report what engines do. The rabbit hops; the logs are clear.

[Medium impact] The PR appears safe to merge; no new blocking issues were found.
Summary
Adds optional operation timing, cleanup progress, and error context for hosts.
Engine operations now send hosts timed success and failure reports.
Server errors now name the workspace, caller, operation, and route.
The mention backfill reports each page and when it finishes.
No new actionable issues were found. Tests were inspected, not run.

The hosted gateway's D1 overload investigation could not attribute load:
engine errors carried no workspace, caller or operation, and nothing
reported how long the expensive engine operations took.
- New optional `EngineDeps.diagnostics` (`EngineDiagnostics`, ports/
diagnostics.ts) receives operation timing for `inbox.get`,
`deliveries.list`, `workspace.delete`, `invocations.sweep` and
`backfill.mention_fts`, plus backfill progress events. Callbacks are
isolated and absent by default.
- Labelled operations run in an AsyncLocalStorage scope;
`currentEngineOperation()` lets a host database wrapper attribute each
statement's cost (rows read/written) to the operation in flight, which
the engine cannot see through drizzle. `configureEngineDiagnostics()`
covers maintenance callers that never build the app.
- Errors thrown inside an operation carry a non-enumerable label, and
app.onError adds workspace_id, agent_id, engine_operation and the
matched route template to its error log and captureException context.

…sync sinks
Review follow-ups:
- createEngine no longer sets a process-wide sink. Each app runs its
requests (and the background work they start) inside
runWithEngineDiagnostics with its own deps.diagnostics, so two apps never
report to each other's sink. configureEngineDiagnostics is now only the
host's explicit fallback, for maintenance outside a request and apps
built without a sink.
- Operation labels live in a WeakMap instead of a defineProperty on the
error, so a frozen/non-extensible error is rethrown untouched with its
status and code, never replaced by a TypeError.
- Sink callbacks may be async: a returned rejection is caught without
awaiting, like safeTelemetrySink, so it can never surface as an unhandled
rejection.

asCodedError wraps a plain-object rejection in a new Error; the label
stayed on the original object, so captureException lost
engine_operation. The wrapper now inherits it.

Preserve the operation label for primitive and array rejections.
The /v1/inbox path reaches withEngineOperation('inbox.get', ...) and awaits the host-injected EngineDb. If that adapter rejects with an array or primitive, asCodedError creates a new Error without the operation label. engine.ts then omits engine_operation from the 5xx log and telemetry context.
Normalize these rejection values at the operation boundary and label the normalized error.

In the catch block of withEngineOperation, normalize primitive and array
rejections into an Error before assigning the operation label; preserve
non-array object rejections. Use the normalized error for operationLabels,
telemetry errorName, and rethrowing so downstream error handling retains the
operation label.
