fix(engine): agent-scoped index for status-filtered delivery listing - #576
Part of the hosted-gateway D1 overload investigation (relaycast-cloud, Oct 22–25). listDeliveries with an explicit status (GET /v1/deliveries?status=dead_lettered, acked, …) filters one agent's rows by status and orders by (created_at, id). The only index serving that order is idx_deliveries_status (workspace_id, status, created_at), which isn't agent-scoped:
SEARCH deliveries USING INDEX idx_deliveries_status (workspace_id=? AND status=?)
USE TEMP B-TREE FOR LAST TERM OF ORDER BY
The query walks every row with that status in the whole workspace, throwing away other agents' rows, before the LIMIT applies. On the hosted gateway about 4M of about 6.5M delivery rows are dead_lettered, so one call can scan millions of rows on the shared D1. (The default no-filter path is already bounded by idx_deliveries_agent_active_created.)
What
Migration 0076_deliveries_agent_status_created_index.sql adds idx_deliveries_agent_status_created (workspace_id, agent_id, status, created_at, id). That's an agent-scoped seek already in the requested order, so the read stops at the LIMIT. The index is also declared in schema.ts.
Why not reuse idx_deliveries_agent_status_seq: it orders by seq, which is not the API's order for historical rows. Migration 0029 renumbered active rows above settled ones, so switching the ORDER BY would silently reorder results.
Changelogs: a Patch entry in both the root and engine changelogs.
Tests
New conformance test: for every status, the plan uses idx_deliveries_agent_status_created and has no TEMP B-TREE. It fails without the migration.
taskMigration.test.ts's pinned applied-migration list includes 0076.
npm run typecheck, npm run lint and the engine suite (1,518 tests) all pass.
Rollout note
Building the index is one pass over deliveries (about 6.5M rows on the hosted gateway) during migrations apply. Like 0074, it changes no data, but apply it off-peak. The hosted worker copies this migration verbatim when it bumps the engine.
`GET /v1/deliveries?status=<status>` filters one agent's rows by status and
orders by (created_at, id). The only index serving that ORDER BY was
idx_deliveries_status (workspace_id, status, created_at), which is not
agent-scoped, so the read walked every row with that status in the whole
workspace and sorted the tie-break in a temp b-tree. On the hosted gateway
~4M of ~6.5M delivery rows are dead_lettered, so one agent's
`?status=dead_lettered` read could scan millions of rows on the shared D1.
Migration 0076 adds idx_deliveries_agent_status_created
(workspace_id, agent_id, status, created_at, id): an agent-scoped seek
already in the requested order, which stops at the LIMIT. A conformance
test pins the plan for every status.

The change adds the idx_deliveries_agent_status_created index to the engine schema and migration. Conformance tests check its query-plan use for five delivery statuses. The changelogs describe the index and the reported change to status-filtered delivery listings.

Summary
Define and register the delivery index packages/engine/src/db/schema.ts, packages/engine/src/db/migrations/*, packages/engine/src/db/__tests__/taskMigration.test.ts
Adds an index on (workspace_id, agent_id, status, created_at, id) to the schema and migration. Updates the expected migration list.
Verify query-plan use and document the change packages/engine/src/__tests__/conformance/delivery.test.ts, CHANGELOG.md, packages/engine/CHANGELOG.md
Checks five delivery statuses for use of the index and absence of a temporary B-tree sort. Updates the unreleased changelog entries.
The query-plan test should match the production projection before merge to avoid missing a performance regression.
